A few people from a Salesforce user group asked my mom to launch the group's first ever salary survey. Mom said yes, but I quickly decided to take over the whole process for her. For my mom's group, the decision was made to do an anonymous survey, with no "required" questions so a person could answer as few or as many as they liked.
